Pre-Participation Information

 

For athletes looking to participate in JH Softball this winter:

Remember that you must have all paperwork turned in, including the following:

  1. CPA Participation Forms  -  Click on the link to submit online.
  2. AIA Physical (required every calendar year)
    • Physical (Pg 1)
    • Physical (Pg 2) 
    • Physical (Pg 3)
  3. Fees:  Fees for Fall 2012 junior high sports are $165 + $15 Athletic Trainer Fee. (Online payment is not currently available.) Please make out our check (no cash please) to Chandler Preparatory Academy and include your student's name and sport (i.e., JHS Football) in the memo field.) Athletic Trainer's Fee - Bottom line we had to make a choice between charging a $15 Athletic Trainer Fee (less than a typical co-pay) and keeping our certified atheltic trainer (Bobby Fennell) on staff, or not charging the fee and losing the position. For numerous reasons we chose the first option. The year every HS and JHS participant in athletics will pay the participation fee for their sport and an additional $15 Athletic Trainer. When submitting your fee, please write two separate checks, one for the sport and one for the Athletic Trainer Fee.
  4. A copy of your insurance card on file with the athletic office.

*Note: If you played a fall sport and all you paperwork was turned in, you only need to pay the fee*

 11-17-2012 13-4 Loss  Anthem Tatum had a great game pitching her first start.  She had 13 K against only 6  walks. Also, Sydni had her first game catching and as the game progressed, so did she.  Great job both of you.
 12-1-12  10-8 Win  Mesa  We had our second game to day and we won.  We not only won, but we did by overcoming an 8 - 2 deficet.  We had a very slow start buyt in the last 2 innings, our bats came alive and our defence worked. In the last inning played (the 5th) Lauren got on base thru an error and scored, Tara had a double and then scored thru Tatum's double and Tatum scored on Sarah's walk-off double
 12-8-12  12-9 Loss  Glendale

We had our "Mini-Tournament" this morning and we did only fair.  We started off very slow and gave Glendale a chance to get ahead.  Once again our girls did not give up or give in and they kept on battling.  I believe we would have overcome that lead, but unfortunately the tournament games are a bit shorter and we simply ran out of time.  No one girl really played above the level of her teammates, but as a team we stoppped Glendale from gaining any more momentum.

 12-8-12  6-6 Tie  Trivium In the second game of our turnament we played Trivium, and although it was a better played game, The Titans were again slow getting out of the gate.  One thing that this tournament prooved was we can stay competitive with any team.  Tatum kept us in the game, and we had some heads up play from Tara and Emma. Although these games do not reflect on our overall record, Titan Pride kicked in and we never gave up.  Nice going girls! 
 1-12-13  10-11 loss  Trivium  This was the second time we played this team and for the second time is was a good one. Although we scored first with 3 runs, our defence was not up to par and we committed 5 errors which lead to 6 runs. Not a pretty sight.  Tatum Vilaboy kept in the game the whole time and had 6 Ks with only 4 walks.  NICE JOB TATUM !!.  We had Emma Merrill catching for her first time in a real game ans she was outstanding.  EWven the Trivium coach came over to say "good game".
 2-4-13  12-14 Loss  Veritos  We has a loss this afternoon but it was a "good" loss.  We had some good takeaways from this game.  Our hitting came alive and we managed to put some hit balls into the outfield.  Olivia went 3 for 3. Emma was 2 for 3, Tatum was 1 for 1 with two walks.  We just issued too many walks.  It was an off night for Tatum who issued 14 walks.  Emma has her good game at catcher and Olivia had three or four assisted put-outs.  We hsd two more games before tournament and I think we will be very competitive.
 2-9-13  11-11 Tie ???  Glendale  We had a double hearder today and due an officiating blunder, we ended up in an unofficial tie of 11 to 11.  We had another good hitting day with 8 hits and only 3 Ks.  Good day for our team.  Tara went 2 for 3 with a homerun.  She also had two unassisted put-outs at first.  All of team stepped up and hit well.  One of our player had a very good offensive and defensive day. 
 2-9-13  14-12 Win  Glendale Even though this game does not go in the record books it deserves to be recognized.  The was Tara's first game starting pitching and pitched the whole game.  She had a rocky start but stayed in there for the game.  She did as good job.  With our team hitting well for the third straight game, we help Glendale in check to giveTara the win. GREAT JOB LADIES
 2-14-13  10-8 Win  Trivium  This was a great win for us.  After loosing to Glendale which moved us inot the Looser Bracket, we came back to win this game.  After being down 5 to nothing we came back to tie and go ahead by one and then went ahead to win 10 to 8.  Tatum had 7 Ks against 7 walks and Emma saved several runs by playing a great cathcer.  We hope to do this again on Saturday against North Point for the winner of the Looser Bracket.  TERRIFIC JOB LAST NIGHT LADIES.

Hey Team

Our season has come to an end and has been a great year.  With only 10 players, we prooved that no team could or did dominate us.  We held our own with each game and got better with each game.  You ladies accomplished something that few teams can boast and that is moving from 10 individuals to a team.  GREAT JOB LADIES.  We will have our Season end "party" on Monday at Floridino's from 5:30 to 7:30.  I hope everyone can attend together with parents, uncles, grandparents and anyone who supports our players.  All of you are welcome.  Now comes the hard part - choosing the Titan Player of the Week.  With some atheletes really stepping up to help us along the way, it is very hard.  There are two girls how have worked hard all year and have really tried to put their best foot forward.  They both have done what was asked of them without question.  They both had good a good game on Saturday so the choice is difficult.  However, this week I will nominate Claudia Clayton.

 Did we do good last night or what !!!!.  You guys were awsome. This just shows that no team in this league is better than you when you play as a team like you did last night.  All of you contributed to the win and now we have one more game to play tomorrow against North Point.  I know very little about this team other than they cancelled on us, Anthem beat tehn 13 to 7 in tournament play and they were seeded #5 and we were seeded #6 so our records are similar.  It is very hard to nominate a Player of the Week for last night game.  We had several girls step up to add to our win -- There were at least 4 girls who stepped up so selecting one is very difficult.  Our batteryn worked well in the game so I have to go with a double nomination.  This will be for both Tatum Vilaboy and Emma Merrill.

Hey Team

Nice games today.  We had a double header against Glendale and although we did win the first game (as well as the second), thru an officiating blunder, the first game ended in a tie.  We did hit the ball very well in both games and Tara had a grand slam to help us in the "victory" of the first game.  Tatum did a great job keeping us the game the whole time in the first game.  Tara pitched the second game and this was her first time pitching the whole game.  Nice job from both girls.  This week's nomination goes to a player who has been to every preactice and has played positions that she was not comfortable but played thee anyway.  She had a good day at the plate and had a good day defensively.  This week I nominate Lauren Peake.

Just a couple of quick reminders.  1)  There will be pitching practice tomorrow afternoon (Friday) at Shawnee starting at 3:30.  Saturday is supposed to be a very cool day weather-wise so dress warmly.  2)  The second game will be a practice game only and will not count in our standings.  Please make sureyou areat the fields on time.

PLEASE READ THE INFORMATION REAGRDING YOUR PICTURES

OK now for our business.  North Point contacted our AD and announced they would not be able to make Fridays game.  This will be a cancellation not a postponment due to the tournament next week.  One point here is this is North Point's issue, not our school's.  So..we will have our game on Saturday per the schedule.  Also, there will be no pitching practice tonight but we will pick it up again on Friday afternoon right after school at Shawnee.  If any of you have any questions, please let me know.

2-5-13:   From the Athletic Department

Winter Sports Pictures are scheduled to be delivered by LifeTouch Sports Photography to our campus this week.  They will be distributed to head coaches when they arrive and then be distributed to the students.  Students who took an individual picture, but did not order pictures on picture day will receive a small photo with instructions on how to order or you may contact Carolina Fuentes (7am – 3pm M-F) at  This e-mail address is being protected from spambots. In addition, I was able to schedule a “make-up’ picture time with LifeTouch on Thursday, February 21 @ 3:45pm.  This is primarily to reshoot the JHS girls CPA 2 soccer team photo, but the photographer will also take individuals for those that now want individual photos but did not sit for them the first time around.  Picture packets (order forms) for “make-ups” will be available from the Athletics office (located next to the gym) approximately a week prior to the photo date.

We had tough loss tonight.  Our pitching was not up to par and we issued 14 walks.  We did, however, have some good takeaways.  Olivia Vilaboy went 3 for 3 and Emma Merrill had a great game catching and went 2 for 3 at the plate.  Although Olivia is helping us out due to injuries, she has really stepped up and is doing well.  She is a credit to the team.  Our game for next Friday has been moved from Palma park to Evergreen Park in Mesa.  This will reduce our travel time greatly.  It will start at 5:00 pm so we need to be there at 4:00 pm.  The Titan Player of the Week nomination goes to an athlete who has come on board only recently and hs really stepped up to help our team.  Although she is very quiet, does not say much she just goes about her business of playing softball.  This week's nomination goes to Olivia Vilaboy.

Here is the latest.  Payton DuBose sprained her ankle and per her Mom, will probably miss the next two weeks.  Sydni's mom said she has brought her grades back up will bale to rejoin the team and with the help of Olivia Vilaboy, we will have 10 players. If Olivia cannot help will be down to nine.  WE CAN DI IT WITH 9 but we will all have to step it up.  We have only three games before tournament so all need to be at every practice and game.  My nominationfor the Titan Player of the Week goes to an athlete that works hard at every practice, shows up to every practice and has improved beyond my expectations.  She has her family support at every game.  This week goes to Tatum Vilaboy.

Well, we have hit another snag in our game schedule.  The city of Phoenix has declaired the fields at Palma Park unplayable for both softball and baseball bacause of the rain this past week.  If you will notice the TENTATIVE schedule fore the make-up game is for Thursday at 6:00 pm.  It came to may attention this afternoon that Thursday is also the 8the Grade Open House so there is a potential conflict.  I will send a notice to our AD to make sure he is aware of the potential conflict.  So.....Stand by for changes.  I also want to thank our AD for the hard work he and Andy have put into trying to get a new place and time for our game.  The way it stands now is the TENTATIVE schedule is for us play a game on Monday at 3:30 at McQueen, Thursday TBD and next Saturday at Palma.

Some quick reminders.  Our game this Saturday is scheduled to start at 9:00 am.  It will at St. Agnus school.  Please be there no later than 8:00 am.  We will be playing Veritas  so be ready for a good game.  The weather forecast is for cool temp. and possible rain so come dressed for cold weather.  As always, just do your best and have fun.

We had our second game against Trivum and for the second time it was good (after the first inning).  Our first inning was not good with 5 errors leading to 6 runs.  Tatum Vilaboy kept us in the game the whole time with 6 Ks and only 4 walks.  Emma  Merrill was catching in her first real game and she was outstanding.  It is hard to nominate the Titan Player of the Game with two athletes doing so well but I have to go with Emma Merrill as the player of the game for this week.

We had our practice yesterday and unfortunately we were a few players short the only problem is that with a couple of players, it was a "no call no show".  Because we have such a small, close knit team, there is big hole when someone misses practice.  I urge all my players to make sure your homework and class projects are up to date and done on time.  This way, there will be minimum missed practices AND your grades will stay GOOD.  I ask all players and parents, if you are going to miis practice, PLEASE let me know.  A "no show" is not very respectfull to the team or to me.  Please give me a days notice if you will miss practice.  We start our "second half" this coming Saturday and this is the beginning of the seeding for tournament.  See you ALL on Monday for practice at McQueen at 3:00 (because of early release).

Wow -  Jan. 1st 2013.  Hard to believe.  And what's even harder to believe, SCHOOL STARTS AGAIN - UGH !!.  Anyway, that is the bad news, the good news is that the practice scheduled for this Thursday has been moved back to Shawnee from 1 to 3.  This is because this s a short day for all of you.  PLease be sure you are there on time.  WE hav a lot of work to do.

The Christmas holiday is over and I hope all of you had fun and "Santa" brought what you wanted most.  Practice starts again this week on Thursday and Friday at Shawnee from 1 to 3.  Thursday will be for the full team and Friday will be for pitchers and catchers only.  Next week after school starts, we will be back at our normal time and location.  We have a game on Jan 12th.  It will an away game.  Please see the shedule for location.

Well today is the last day of exams.  I hope everyone did well and no one gets "left behind".  To those who had to miss practice because because of grades, we missed you and hope everything went well.  Our practice for this coming week will be held at Shawnee on Monday, Tuesday and Thursday.  Practice will run from 1:00 to3:00.  There will be no practice on Dec. 24th, 25th, 31st or Jan 1st.  There will be practice on Dec. 27th and 28th from 1 to 3 at Shawnee.  Location and time for practice on Jan. 3rd to be determined.

In may haste to get everything done yesterday, I forgot to announce my nomination for the Titan Player of the Week.  The JHS Softball nominee this week is an athlete who is always "up".  She is always keeping the other girls positive with her encouragement and cheers.  She is one of the hardest working players on our team and any GHMSL coach would like to have her on their team.  Tara Meagher is our nominee for Titan Player of the Week.

As I have mentioned, the practice schedule for next week has been changed because of finals week.  Monday is a standard full class day and our practice will stay as scheduled (3:30 to 5:00) at McQueen Park.  Because the girls get out at 12:45 Tuesday through Friday, we will have practice at Shawnee Park from 1:00 to 3:00 on Tuesday and Thursday.  Please make sure the girls have carpool to and from the field.  We will also be looking at the practice schedule during the Christmas break.  More news on that later.  Please stay close to this Blog.  Thank you for your support for this team and for the support for the individual girls.  I appreciate it.

Hey Team

Well I finally got the schedule for our tournament for next week-end.  It will be a "mini" tournament and will not reflect on our overall won/loss record.  However, we want to make a good showing at the games to give notice to our competitors.  We will have two games, both played at Palma Park.  For those who do not know where that is, it is located on the corner of 12th Street and Dunlap in Phoenix.  Our first game is at 8:00 and has a one hour time limit.  Our second game is at 10:00, again with a one hour time limit.  Please make the arrrangments for carpooling to and from.  If you need help, please let me know.

Hey Team

We had our second game today and it was a dandy.  Once again our team did not give up or give in and to show that, we won by overcoming an 8 to 2 defecet.  We stared of very slow and was not until the 3 inning did we finally pick up.  Our hiting finally started to show and our defence was there also.  Tatum did another good job and we had two double plays.  It was fun to watch.  In the 5th inning, we were still two runs down when Lauern got on base on an error, stole second and scored on Tara's double.  Tatum came up to bat and shot a double to center field and Tara scored.  Tatum then got to third on a passed ball and then scored on Sarah Yang's WALK-OF DOUBLE.  That was Sarah's second hit of the game and because of her attitude in practice, her coachability and her outstanding game, I'm nominating her for the "Titan Player of the Week.

Following our new program, I will nominate one of our athletes from last week's game.  She did an outstanding job as her time as a starting pitcher.  She kept her poise and never gave up.  She had 13 Ks with only 6 walks.  She showed leadership in practice her attitude was outstanding.  Tatum Vilaboy is our first nominee.  Congratulations Tatum !!!

I hope everyone had a great Thanksgiving with your family and friends.  As we did have a game this weekend thee is not a lot of to bring you up to date on.  There one thing I really do need to remind all of you about and that is reading THIS BLOG at least twice a week.  Any game or practice changes will be shown here, so please make sure you are up to date.  Our regular practice schedule begins again this week and we have a game this Saturday.  Please check the online calender for time, place and opponent. A map can be obtained by clicking on the Google calendar item. The Titan Athlete of the Week program starts this week.  JHS softball will have one nominee each week and that nominee witll have the chance to be named the “Titan of the Week”.  The JHS softball nominee selected will achieve three things: 1) Playing Titan softball in the weekly game 2) showing leadership not only at the game but during practice that week,  and 3) Good overall attitude.  The coaching staff will take all three factors into consideration before putting forth a nominee. The girl's name will be published in TitanPride, the weekly athletics newsletter.  See you this afternoon at practice.

Well we had our first game and it was a good one.  We lost to Anthem (13-4) but asd I told you before the game, I was not looking for the final score, I was looking for effort and AS A TEAM you guys came through.  You did not give in or give up and it was a great thing to see.  I have to take my hat off two of our players.  One is to Tatum.  This was her first time starting pitching and she did an outstanding job.  He had 13 Ks with only 6 walks.  She kept her composure and kept throwing strikes.  The other is to Sydni.  Her first time a starting catcher and as the game progressed, so did she.  By the end of the games she was looking like a verteran.  I couild go on about the rest of our team like our first baseman, our center fielder but I don't have the room.  I'm excited about what I saw.  We have work to do but we will get there.  NICE JOB LADIES.

Hey team.... Saturday molrning is our first game and I'm getting very excited about starting our season.  I'm sorry about having a short practice week before the game but it could not be helped.  What I need now is "all hands on deck".  What I mean by that is, I need to have all of you at both practices in order to assemble our team and place all players in the correct positions to do the best for our team.  We are playing Anthem.  Please remember that at 7:00 am it is cold so dress warmly.  Also, when you get sleeves, please make sure they are black and you have red socks.  The jerseys will be handed out on game day, so plan on getting there a little early.

Heads-up.  For next week, we will have only two practices because I have a family issue that cannot be changed.  So... our practices will be on Thursday (11-15-12) at our normal time and place and on Friday (11-16-12) normal time and place.  One other issue - OUR FIRST GAME IS SCHEDULED FOR SATURDAY 11-17-12.  Please check the calender for games and practices for maps.  Our games will be at St. Agnes school at 8:00 am so need to be there one hour before game time.  Any questions, please fel free to contact me.

Even though our team is not very big, we have some good athletes and our rookies are showing good promise.  Our pitching is coming along better than I had anticipated and the hard work and enthusiasm the team has shown makes me excited about we can accomplish this year.  Since we have to off the field by 5:00, I'm asking that that all of you do what you can to get to the field by 3:30.  The later you get there, the less time we have to practice.  Also, if any of you know of someone who wanbts to join our team, please let me know.  As soon as I get the game schedule I will let you know.  It will be posted under the calander of games and practices.

For those of you who do follow the Blog, please make sure you follow the instructions under the "Pre-Participation" tab.  If you have already turned in the paperwork for your physical, then all you need do is get the check in to the front office or to Mr. Polley.  If you need directions to McQueen Park, please go the "Schedule and Stats" tab and pull up the calender and look under Map and it will give directions. The prctice field is not with-in walking distance, so a ride (car-pool, etc.) needs to be arranged.  Last year's team had at least one game at these fields so for some of you it should not be an issue.  See you all on Monday at 3:30 at McQueen Park.

Happy Hollween to all of you and be careful if go out tonight.  Our first practice is right around the corner and I hope all of you are ready.  Your coaching staff will be evaluating you for the first two or three practices to see where we stand.  If my intuition is correct, we should have a strong team.  Please make sure you have all of your equipment (shoes, glove, etc.) ready to go.  As soon as the game schedule is available, I will let you know.

Hey Team - Well our first practice is getting closer and I want you to know that the coaches will be evaluating each athlete on the following: 1)athletic ability 2)attitude 3) participation 4) learning ability.  All of these attributes and more are what the coach will be looking for.  Like we talked aboput in our meeting, part of your job is to make my job of picking out the line-up difficult.

Just a quick note to say WHAT A FANTASTIC TURNOUT FOR TODAY'S TEAM MEETING.  I am really excited about the possibilities for this year's team.  Way to go LADIES !!!  Please remember to get your physicals as soon s you can and turn in your sport participation fee and the trainers fee to Mr. Polly or to the front desk.  Please remember to read the blog everyday.

Practices:  Practice will be held at Shawnee park from 3:30 to 5:15 on MondayNovember, 5th.

 Here are a few items to get started with JHS Girls Softball:

Primary Communication: I will be utilizing the Chandler Prep website Coach's Blog as primjasry communication.  Please make sure you check the Blog daily.  Emails will be reserved for changes from what is communicated through the blog or emergency communication. Practices and game information will be posted to the website on the JHS Girls Softball sports calendar accessible online on the JHS girls softball page of the chandlerprep.org website under the “Schedules” tab. E-mail will only be used for non-routine circumstances.

Once you've read through the information below feel free to email me at the email listed on this site with any questions, comments or concerns. I will work very hard to reply to your emails as soon as I can.

AIA Physicals:  Please print off the 3 AIA Pre-Participation Physical forms listed under the “Participation Information” tab, and if needed have your physical conducted for the 2012-13 season.  Each athlete must have all three (3) pages of an up-to-date (completed after 3-15-12) physical  for the 2012 season on the pre-printed AIA form completed andonfile with the Athletic Department prior to the day of our first practice (Monday, November 5th, 2012).  Please note that Page 2 of the form must be completed by a physician.

*** If you do not have an up to date physical on file at school, and are planning on going to your primary physician for one, be aware that in some cases you may not be able to get an appointment with your family doctor for a physical for several weeks. Please note that your athlete will not be allowed to practice until their physical is completed. Missing practice and tryouts will most likely result in your athlete not being put on a team. Keep in mind that there are other places, like Fry's or any Urgent Care, that will complete same day sports physicals for $25 - $30. Remember to bring your signed AIA forms to the Athletic Department before the first day of practice.

On-line forms:These forms can be found under the Pre-Participation tab and must be filled out on-line (takes about 2-3 minutes).

Copy of Insurance Card: Each athlete must provide either a hard-copy of the player’s family insurance card (FRONT AND BACK), or a written and signed note from their parent’s to the effect that they do not have health insurance.

Payment of Participation Fee:The fee of $165 must be paid or payment arrangements made PRIOR TO THE FIRST PRACTICE.  This is a new policy and it will be enforced.  Those athlete/families that have applied for FAIR waiver should note that athletic fees are not automatically waived even if you’ve applied to FAIR.  If you need to discuss a fee waiver contact Mr. Polley directly via email at This e-mail address is being protected from spambots. Dress Code: Please come to practice each day in proper attire. Proper attire includes is different for different sports but should include: softball cleats or running shoes (NO Converse, Vans etc, gym shoes or school shoes), sliding shorts and softball pants, athletic t-shirt (Chandler Prep shirts preferred),  a glove and a bat (1st is required, 2nd optional).

Athletes will not be allowed to participate in practice if they do not wear the proper shoes.  If your family is not able to provide a glove, please let coach know ASAP as we may be able to help you solve that problem for the season.

Camps: There is no CPA preseason JHS softball camp.  

Which Team/How many teams? Our JHS sport turnouts continue to grow, and while we’d like to provide every athlete that desires to compete for their school a chance to do so the league structure and finances may not allow us to do so.  Some sports will be limited to a set number of athletes (volleyball A & B, baseball, softball, basketball A & B) while many others will not face such limitations (football, x-country, soccer, track).  Currently we believe the turnout for softball will be such that we will offer one team and not have to “cut” anyone form participation.  However, in any case an athlete that does tryout for one team, fails to make that team, and desires to transfer their fee to another sport (i.e., one without limitations on enrollment)  can do so without penalty.  Please contact Mr. Polley ( This e-mail address is being protected from spambots. You need JavaScript enabled to view it ) directly with this request and he will walk you through the process.  In matters related to tryouts the coach’s word/decision will be final.

** Athletes that do not make a Chandler Prep team and do not desire to transfer to another sport will have their fee refunded.

Tryouts/Evaluation:  As one team there will not be a need for “tryouts” for JHS softball.  Instead, assessment of players skills will be ongoing beginning with practice on Monday, November 5th.  The coaching staff will utilize an both objective and subjective criteria to assess each player's abilities, teamwork, communication, drive, motivation, energy, attitude, coachability, etc. 

Team Structure: I will address all issues such as absences, tardies, academics, games, practices, uniform, conduct, attitude, and sportsmanship during the first week of practice.   

Volunteer Opportunities: I will be asking for volunteers for games for many jobs.  A complete list of those opportunities will be discussed during the parent/athlete meeting.  Date TBD.

 Parent involvement is vital for us to provide the experience your athlete deserves. Your athlete will really appreciate your support at games and it will be fun for you to play a role in their Titan athletic career.
